Salvation Army “back2school” application

The Salvation Army in Galesburg is taking applications for “back 2 school” supplies July 31, 2017 from 2-5 PM for children in Kindergarten through 8th grade.
You must bring the following:
*Picture ID (Parent listed on medical card or lease)
*Proof of residence living in Knox County (Lease or utility bill)
*Proof of children in the household (Medical card or birth certificate)
Back 2 school supplies will be distributed on August 12,2017 from 2-5 PM.
